Lichen Sclerosus (LS) is a benign but chronic skin condition commonly found on the genitals and characterized by white patches that usually causes itching, burning or pain. In woman, LS can lead to thinning and shrinkage of the genital area that can make intercourse, urination, and defecation painful. Previous treatment options were limited to steroids. Vaginismus is a condition affecting a woman’s ability to engage in any form of vaginal penetration, including sexual intercourse, insertion of tampons, and the penetration involved in gynecological examinations. An involuntary reflex of the pubococcygeus muscle causes the muscles in the vagina to tense suddenly, which makes any kind of vaginal penetration painful or impossible. The vaginismic reflex can be compared to the response of the eye shutting when an object comes towards it.
A variety of factors can contribute to vaginismus. These may be physical or physiological, and the treatment required depends on the individual. A pelvic physical therapist can assess hypertonic pelvic muscles that often affect and/or cause vaginismus.
Treating Vaginismus at Fox Valley Plastic Surgery
Using BOTOX® injections to treat vaginismus dates back to 1997. BOTOX and a local topical anesthetic are injected in the vaginal walls, and the vagina is then progressively dilated after a physician applies the anesthetic. Under the supervision of a pelvic physical therapist, the patient practices moving the dilator in and out of the vagina for several days.
