If you have an virtual appointment, click on the correct image below to enter the “waiting room” at your appointment time.

If have an impending visit, please click on Patient Forms to the right to download the necessary patient forms prior to your visit. Having them filled out ahead of time can easily save you 15-30 minutes in the waiting room. If you have been in the office within the last 12 months, your paperwork is probably current, and you do not need to fill out the paperwork. The receptionist who took your appointment, will be able to tell you if you need to update your paperwork. There may be more procedure specific forms, such as consents, for you to fill out in the office, but they are quick.

To view our upcoming events and seminars, please click on Patient Seminars to the left. You may enroll online or via telephone. Calling us on the phone gives you instant confirmation. Some events require a fee or deposit, so calling us on the phone works the best for those events. However, you are free to enroll online too.
